Background Arctium lappa L. ( A. lappa ) has been used in traditional medicine worldwide and is increasingly being investigated for its immunomodulatory and anti-inflammatory effects. However, its therapeutic relevance for immune-mediated skin diseases (IMSDs) remains incompletely defined. Objective This review critically evaluates experimental and clinical evidence on A. lappa and its major lignans, arctiin and arctigenin, in IMSDs, including those associated with atopic dermatitis (AD), psoriasis, systemic lupus erythematosus (SLE), alopecia, systemic sclerosis (SSc), and vasculitis. Methods We systematically searched PubMed, Web of Science, and Scopus up to July 2025 using defined keywords. Eligible studies included in vitro , in vivo , and clinical investigations assessing the immunological and dermatological outcomes of A. lappa extracts or purified metabolites. Results Preclinical studies have demonstrated that A. lappa extracts and their lignans modulate key inflammatory pathways, including the NF-κB, JAK/STAT, and NLRP3 inflammasome signaling pathways. Evidence indicates protective effects on keratinocyte hyperproliferation, mast cell activation, dermal fibroblast fibrosis, and vascular endothelial inflammation. However, most data are derived from in vitro or murine models using heterogeneous preparations, with limited clinical validation. Reported doses range from 10-100 μM in cell assays to 15-100 mg/kg in animal studies, but pharmacokinetic and safety data remain insufficient. Conclusion A. lappa shows promising immunopharmacological potential for IMSDs, but the evidence remains preliminary. The current literature is limited by variability in extract preparation, a lack of standardized dosing, and the absence of robust randomized clinical trials. Future research should prioritize standardized phytochemical characterization, translational animal models, pharmacokinetic studies, and controlled clinical investigations to establish efficacy and safety.

Following a request from the European Commission, the EFSA Panel on Additives and Products or Substances used in Animal Feed (FEEDAP) was asked to deliver a scientific opinion on the safety and efficacy of a dried extract prepared from the roots of Arctium lappa L. ( A. lappa dry extract) when used as a sensory additive (flavouring compound) in feed for cats and dogs. A. lappa dry extract is specified to contain at least 2% inulin. Since uncertainty remains concerning the nature of up to 77% of the additive, the FEEDAP Panel was unable to conclude on the safety of the extract at the proposed use levels of up to 40 mg/kg complete feed for cats and dogs. In the absence of data, no conclusions can be drawn on the safety for the user. In the absence of convincing evidence that the extract acts as a flavour in animal feed or has an effect on palatability, the FEEDAP Panel was unable to conclude on the efficacy of the additive.

Objective To evaluate the acceptability, tolerability, and effects on vulvovaginitis symptoms and signs of a non-soap, herbal-based intimate solution (Zelesse®). Methods We conducted a prospective, observational, multicenter study including adult women with symptoms and signs of vulvovaginitis with various etiologies, including candidiasis, trichomoniasis, bacterial vaginosis, and atrophic and irritative vaginitis. The presence and intensity of signs (edema, erythema, vaginal discharge) and symptoms (pruritus) of vulvovaginitis were evaluated before and after 5-15 days of daily use of Zelesse® alone or as a coadjuvant in antimicrobial therapy. Variables following a normal distribution and categorical variables were analyzed using the Student t-test and chi-square or Fisher's exact test, respectively. Results A total 137 women were enrolled in the study; 87 (63.5%) women received concomitant antimicrobials and 50 (36.5%) used Zelesse® only. Global symptom scores and frequency of patients with vulvovaginitis signs and symptoms, and their mean intensity, decreased after treatment in both patient groups. Vaginal pH and (in the Zelesse®-only group) vaginal flora remained unaltered. The product was safe, well tolerated, and highly accepted by patients. Conclusions Zelesse®, the non-soap herbal-based solution in this study, may represent a safe and effective option for symptomatic relief of vulvovaginitis.

This study aimed to investigate the in vitro antifungal effectiveness of five different formulations against dandruff and ringworm dermatophytes. Candida albicans was also included in our assays. Fungal susceptibility tests were performed with planktonic cells and biofilms of reference strains. Microbiological and physicochemical quality parameters were assessed for all formulations. Our data indicated that the formulations were effective against the dermatophytes strains, and to our knowledge, the effectiveness of cosmetic formulations against fungal biofilms is shown for the first time. The formulations were considered effective against the explored dermatophytes and were considered safe given the adequate microbiological and physicochemical characteristics shown in the proposed assays.

In the present investigation the changes on physical stability (pH, viscosity, flow index and tixotropy) of topical formulations were evaluated following inclusion of different plant extracts containing flavonoids. Also, the antioxidant effect of these plant extracts alone and after addition in the formulation was evaluated using chemiluminescence and the stable free radical 1,1-diphenyl-2-picrylhydrazyl (DPPH(.-)) assays, as well as the inhibition of lipid peroxidation. Formulation added with dl-alpha-tocopherol was used to compare the physical stability and antioxidant activity. Formulations with plant extracts showed pseudoplastic behavior with decreasing on viscosity and tixotropy. The Glycyrrhiza glabra (GG) and Ginkgo biloba (GB) extracts alone and the formulations containing these extracts showed great antioxidant and free radical scavenging activities while the other extracts studied (mixture of Glycyrrhiza glabra, Symphytum officinale L and Arctium majus root, Nelumbium speciosum and soybean) showed lower activity. The results suggest that GG and GB extracts may be used in topical formulations in order to protect skin against damage caused by free radical and reactive oxygen species.
